In line with this goal, Healthy People 2030 features many objectives related to SDOH. These objectives highlight the importance of &#8220;upstream&#8221; factors, usually unrelated to health care delivery, in improving health and reducing health disparities.<\/li>\n<li><a href=\"https:\/\/www.healthypeople.gov\/2020\/topics-objectives\/topic\/social-determinants-of-health\" target=\"_blank\" rel=\"noopener\">Healthy People 2020<\/a><br \/>\nThe Social Determinants of Health topic area within Healthy People 2020 is designed to identify ways to create social and physical environments that promote good health for all.<\/li>\n<\/ul>\n<h4>Rural Health Information Hub<\/h4>\n<ul>\n<li><a href=\"https:\/\/www.ruralhealthinfo.org\/toolkits\/sdoh\" target=\"_blank\" rel=\"noopener\">Social Determinants of Health in Rural Communities<\/a><br \/>\nProvides evidence-based models and resources to address social determinants of health in rural communities.<\/li>\n<\/ul>\n<h3>Screening tools for social determinants of health<\/h3>\n<h4>National Association of Community Health Centers<\/h4>\n<ul>\n<li><a href=\"https:\/\/www.nachc.org\/research-and-data\/prapare\/#:~:text=The%20Protocol%20for%20Responding%20to,patients'%20social%20determinants%20of%20health\" target=\"_blank\" rel=\"noopener\">Protocol for Responding to and Assessing Patients&#8217; Assets, Risks and Experiences tool<\/a><br \/>\nThis tool includes 15 core questions and five supplemental questions.
